We welcome new patients. With few exceptions, we usually require referrals from your primary care physician or a specialist.
Most physicians will provide you with referrals to our practice. If you have difficulties in obtaining referrals, please go to the nearest Doctors Care or Doctors Express urgent care facility to obtain the referral order. Please download this form and give it to your referring doctor. We will be glad to assist you in obtaining necessary referrals from your doctors or from one of who values our service.

Once we have received your referral, we will contact you by phone to set up a convenient time to meet with the doctor. This is your first visit, so please allow 2 hours for your office visit. If you have completed all these forms your stay may be much shorter. Please follow the instructions given here...
Follow these steps to complete our initial new patient information: Save Time!
Step 1: Download financial form , Privacy form , Accident forms ( BCBS, Other insurance) and fill out these forms
Step 2: Let us know where you are hurt, and what you have tried for this problem. Download pain diagram and medication list. We need to know what treatment you had already had for your problem.
Step 3: Click here to enter our online new patient questionnaire. You can enter your medical complaints and information online using our secure link (SSL encryption) and arrive at our office. You will have very little waiting if you complete these 3 steps prior to visit.
There is no typing, just click your choice of answers to a series of computer generated questions. its important you answer all questions, do not skip! if you omit any question, you may need to update again, on our office computers, probably causing some delay! This could take 30 mins.
Please pay attention: if you are claiming disability welfare or work restrictions you need to complete the relevant sections, otherwise we don't have your information to complete insurance forms, claim forms, letter to employer or medical-legal experts that follows your visit.
Please try to complete online before 7am on the day of consultation. Your information is reviewed by medical staff from 7am onwards. Preview of your information helps us to start planning your treatment options. Information entered after 7am will be reviewed at time of consultation.

No problem, just come on time to complete the questionnaire. We have assistants to help you navigate our computer program that will walk you through your medical history. By using a computer mouse, you can click your choice and enter your entire medical history in less than 30 minutes. Dr. Thiyaga will see you immediately after entering all necessary information
